Tell Us About Your Childhood – Survey Closes March 13
We’re inviting you to take part in the Community Well-Being Survey a project led by Public Health Ontario. This survey is designed to better understand how childhood experiences can have long-term effects on your health. The information you share will guide programs and services that make our community healthier and stronger.  The survey is confidential, takes about 20 minutes, and can be completed online. If you received a postcard in the mail, it means your household was randomly selected to take part in the survey. Your postcard includes a unique passcode—enter this code when starting the survey online to access the survey. If you didn’t receive a postcard, you can still participate in the online survey.
